Safe Home Transitions provides home safety evaluations and transition planning services for patients returning home from hospital or rehabilitation. Evaluations are performed by a licensed Physical Therapist specializing in mobility, fall prevention, and home safety.

Accessibility & Modification Recommendations
We provide guidance on home modifications that may improve safety and accessibility, such as grab bars, ramps, stair solutions, bathroom adjustments, and doorway widening. When needed, we can help coordinate trusted professionals to complete recommended modifications.
About the Founder
Safe Home Transitions was founded by Alex Tessman, PT, DPT, a licensed Physical Therapist dedicated to helping individuals recover safely and confidently after illness, injury, or hospitalization.
Through years of experience working with patients during rehabilitation and recovery, Alex saw a common challenge: many individuals return home from hospitals or rehabilitation facilities to environments that are not fully prepared to support their mobility and safety needs.
Safe Home Transitions was created to help bridge that gap.
By combining professional expertise in mobility, fall prevention, and home safety with a compassionate, patient-centered approach, Alex helps families prepare their homes so loved ones can return home safely and confidently.
Her goal is simple: to make the transition from hospital to home smoother, safer, and less stressful for both patients and their families.
